In this research, 57 S. aureus clinical isolates were characterized by their molecular and phenotypical properties: The detection of the mecA resistance gene, the pvl virulence gene, Spa typing, and their antibiotic susceptibility profile. S. aureus isolates were classified as methicillin-resistant S. aureus (MRSA), methicillin-sensible S. aureus (MSSA), oxacillin sensible methicillin-resistant S. aureus (OS-MRSA), and borderline oxacillin-resistant S. aureus (BORSA). MRSA correspond to 47% of the total isolates. The presumptive origin of MRSA infection was mainly related to community environments (57% CA-MRSA). We would conclude that community-acquired (CA-MRSA) isolates are replacing healthcare-associated (HA-MRSA). The presence of pvl was detected in almost every clinical group, (e.g., multi-resistant MSSA isolates), and determine this as an ambiguous marker for CA-MRSA. Finally, the spa-typing results allowed us to identify the possible establishment of pandemic clones in Bogotá-Colombia. The characterization of Staphylococcus aureus clinical types is difficult because of transferable mobile genetic elements that harbor remarkable resistance and virulent genes; nonetheless, their accurate identification is necessary to guarantee therapeutic success.

En esta investigación se caracterizaron 57 aislamientos clínicos S. aureus mediante su perfil antibiótico y molecular. La detección del gen de resistencia mecA, el gen de virulencia pvl y rastreo epidemiológico mediante spa-typing fue realizada. Los aislamientos fueron clasificados como S. aureus resistente a meticilina (SARM), S. aureus sensible a meticilina (SASM), S. aureus sensible a oxacilina y resistente a meticilina (SARM-OS), y S. aureus con resistencia limítrofe a oxacilina (BORSA). El grupo clínico con mayor proporción correspondió a SARM (47% de los aislamientos) concordando con reportes previos. El origen de los aislamientos fue clasificado como infección adquirida en comunidad (SARM -AC) u hospital (SARM-AH), donde el 52.6% de SARM fue presuntivo de origen en comunidad. Por lo anterior, se puede concluir que los aislamientos de comunidad están reemplazando a los de hospital. El gen pvl fue detectado en todos los grupos clínicos, por lo que no puede definirse como un factor de clasificación de SARM- AC. Asimismo, la tipificación molecular del gen spa nos permitió identificar cuáles fueron los clones pandémicos exitosos en Bogotá- Colombia.
