A pesar de que existen estrategias de seguridad del paciente enfocadas en los procesos de reconciliación medicamentosa, tanto al ingreso como al egreso hospitalario, en Colombia son escasos los estudios sobre esta temática y, particularmente, sobre estrategias educativas dirigidas a pacientes dados de alta con este tipo de medicamentos. En este contexto, el presente estudio, de tipo observacional descriptivo con enfoque cualitativo, se propuso diseñar una estrategia educativa impartida por farmacéuticos orientada al uso seguro de medicamentos de alto riesgo en los pacientes dados de alta, con el fin de mejorar el conocimiento sobre su patología y el uso seguro de la medicación, reduciendo así el riesgo de eventos adversos y reingresos hospitalarios. Para ello, se realizó una búsqueda sistemática, seleccionando artículos publicados en los últimos siete años (2017-2024), que abordaran estrategias educativas impartidas por farmacéuticos sobre el uso seguro de medicamentos en el momento del egreso hospitalario. Los resultados evidenciaron que los anticoagulantes, opioides e hipoglucemiantes presentan el mayor potencial de riesgo, siendo los adultos mayores con múltiples comorbilidades quienes más se beneficiarían de intervenciones educativas estructuradas. Con base en estos hallazgos, se diseñó una estrategia integral fundamentada en modelos pedagógicos del constructivismo social, andragogía y aprendizaje experiencial, que incluye componentes estructurales, educativos, instrumentales e institucionales, con indicadores específicos para su monitoreo y evaluación. La implementación de esta estrategia podría contribuir significativamente a la mejora de la seguridad del paciente, la reducción de eventos adversos relacionados con medicamentos y la optimización de la eficiencia del Sistema de Salud.

A pesar de que existen estrategias de seguridad del paciente enfocadas en los procesos de reconciliación medicamentosa, tanto al ingreso como al egreso hospitalario, en Colombia son escasos los estudios sobre esta temática y, particularmente, sobre estrategias educativas dirigidas a pacientes dados de alta con este tipo de medicamentos. En este contexto, el presente estudio, de tipo observacional descriptivo con enfoque cualitativo, se propuso diseñar una estrategia educativa impartida por farmacéuticos orientada al uso seguro de medicamentos de alto riesgo en los pacientes dados de alta, con el fin de mejorar el conocimiento sobre su patología y el uso seguro de la medicación, reduciendo así el riesgo de eventos adversos y reingresos hospitalarios. Para ello, se realizó una búsqueda sistemática, seleccionando artículos publicados en los últimos siete años (2017-2024), que abordaran estrategias educativas impartidas por farmacéuticos sobre el uso seguro de medicamentos en el momento del egreso hospitalario. Los resultados evidenciaron que los anticoagulantes, opioides e hipoglucemiantes presentan el mayor potencial de riesgo, siendo los adultos mayores con múltiples comorbilidades quienes más se beneficiarían de intervenciones educativas estructuradas. Con base en estos hallazgos, se diseñó una estrategia integral fundamentada en modelos pedagógicos del constructivismo social, andragogía y aprendizaje experiencial, que incluye componentes estructurales, educativos, instrumentales e institucionales, con indicadores específicos para su monitoreo y evaluación. To this end, a systematic search was conducted, selecting articles published in the last seven years (2017-2024) that addressed educational strategies delivered by pharmacists on the safe use of medications at the time of hospital discharge. The results showed that anticoagulants, opioids and hypoglycemic agents present the highest risk potential, with elderly patients with multiple comorbidities being those who would benefit most from structured educational interventions. Based on these findings, a comprehensive strategy was designed based on pedagogical models of social constructivism, andragogy and experiential learning, which includes structural, educational, instrumental and institutional components, with specific indicators for its monitoring and evaluation.
