Este trabajo tiene como objetivo caracterizar demográfica, clínica, serológica y terapéuticamente a una cohorte de pacientes con LORA atendidos entre 2013 y 2021 en la Clínica de Artritis Reumatoide del Hospital Universitario Fundación Santa Fe de Bogotá. A través de un análisis retrospectivo, se evaluarán variables como edad al diagnóstico, sexo, manifestaciones clínicas, perfil serológico, comorbilidades geriátricas y uso de Fármacos Antirreumáticos Modificadores de la Enfermedad (DMARDs). Además, se compararán los hallazgos con una cohorte de pacientes con Artritis Reumatoide de Inicio Temprano (YORA) con el fin de identificar diferencias relevantes entre ambas poblaciones. Se espera que los resultados permitan comprender mejor el comportamiento clínico de la LORA, identificar factores pronósticos relacionados con evolución y respuesta terapéutica, y apoyar el desarrollo de estrategias de tratamiento más personalizadas para adultos mayores con esta condición.

As life expectancy increases globally, so is the prevalence of chronic diseases such as LORA, which is characterized by an atypical presentation and complex management due to comorbidities and geriatric syndromes. Given the lack of specific data on LORA in Colombia, this study aims to characterize the demographic, clinical, serological, and therapeutic features of a cohort of patients diagnosed between 2013 and 2021 at the Rheumatoid Arthritis Clinic of Fundación Santa Fe de Bogotá. The retrospective analysis of these data will help identify the features of LORA in the Colombian population, thereby contributing to improving diagnosis, management, and the development of more personalized treatment strategies for these patients. In terms of results, it is expected that the characterization of this Colombian population will reveal specific features that can be compared with with a cohort of patients with Young-Onset Rheumatoid Arthritis (YORA) to identify relevant differences between the two populations. In addition to data on age at onset and gender, the study seeks to uncover the clinical and serological characteristics specific to this Colombian cohort. Furthermore, it is anticipated that this population will show a higher burden of comorbidities and geriatric syndromes compared to patients with conventional rheumatoid arthritis, which could influence the response to disease-modifying antirheumatic drugs (DMARDs). Through these findings, the study aims to identify relevant prognostic factors for the disease, such as progression, treatment response, and quality of life. Finally, the study seeks to compare these data with international LORA cohorts to identify significant differences that may inform adjustments in disease management tailored to the local population and propose guidelines for the clinical management of LORA in Colombia. This may include adapting existing clinical practice guidelines, developing more personalized treatment strategies, and implementing interventions to improve the quality of life of patients with LORA. Additionally, the findings are expected to generate hypotheses for future research on LORA, including studies on disease pathogenesis, the effectiveness of various treatment strategies, and the impact of interventions aimed at improving patient quality of life.
