The problem with IVCFs apart from complications due to migration and breakage of the device is that their long-term use paradoxically increases the risk of deep venous thrombosis (DVT) 2. For this reason, IVCFs should be removed as soon as anticoagulation therapy is no longer contraindicated3. However, the literature on the subject shows that a large percentage of IVCFs are left in place and never removed 4. Additionally, there is an increased use of IVCFs in scenarios where its use is not evidence based such as primary prophylaxis in trauma or bariatric surgery, and in patients submitted to lower limb local thrombolysis or pulmonary endarterectomy 5,6. These practice exposes patients to the risk of iatrogenic thrombotic complications and increase health care costs. No data has been published regarding the use of IVC filters in Colombia. The objective of this study is to evaluate the indications, practices, removal rates, and complications of IVC filters in a Colombian university hospital.

The objective of this study is to evaluate the indications, practices, removal rates, and complications of IVC filters in a Colombian university hospital.RESUMEN: Los filtros de vena cava inferior (FVCI) son dispositivos endovasculares utilizados para reducir el riesgo de trombo-embolia pulmonar (TEP) y muerte en pacientes con trombosis venosa profunda (TVP) proximal aguda y contraindicación absoluta de anticoagulación o con TEP recurrente a pesar de una adecuada anticoagulación 1. El problema con los FVCI es que entre otras complicaciones como fractura o migración del dispositivo, su uso a largo plazo, paradójicamente aumenta el riesgo de recurrencias de TVP 2. Por esta razón, el FVCI se debe retirar tan pronto como la contraindicación de anticoagulación se supere y se pueda iniciar la terapia anticoagulante según corresponda 3. Sin embargo, la literatura muestra que un gran porcentaje de los FVCI nunca se retiran 4. Adicionalmente, hay un uso creciente de FVCI en escenarios no basados en la evidencia, tales como profilaxis primaria en pacientes traumatizados, pacientes que van a ir a cirugía bariátrica, pacientes que van a ser sometidos a trombolisis venosa local de miembros inferiores o endarterectomía pulmonar 5,6. Esta práctica expone a los pacientes a riesgo de complicaciones trombóticas iatrogénicas y genera altos costos para los sistemas de salud. No existen datos publicados respecto al uso de FVCI en pacientes no oncológicos en Colombia.
