Sporadic cases have been described in Central and South America. The objective of this study was to describe the clinical and epidemiological features and ecological characteristics of melioidosis in Antioquia, Colombia. Methods: This is a case series description. Results: Seven cases were identified. Burkholderia pseudomallei was isolated from peripheral blood, pleural fluid, and urine and was identified by the automated system VITEK 2 (bioMe´rieux) and API 20NE biochemical kit. Five of the cases had a bacteremic form with shock and pulmonary compromise and two of these patients died. The non-bacteremic melioidosis cases had genitourinary, abdominal, and osteoarticular compromise. All patients had comorbidities and lived in rural hot and humid areas in the west central region of Colombia (Antioquia). Diabetes mellitus, renal insufficiency, and other chronic diseases are important risk factors for the development of severe forms. Conclusions: The cases presented here are similar to those occurring in endemic areas regarding comorbidity, risk factors, clinical presentation, and environmental conditions. It is necessary to establish whether melioidosis is an endemic and under-diagnosed disease or an emerging disease in Colombia. |

La incidencia en Tailandia es de 4,4/1 000 000 de habitantes, donde causa el 19% de las neumonías adquiridas en la comunidad (NAC) y el 20% de las neumonías bacteriémicas, y la mortalidad es del 50%. En América Central y del Sur se han descrito casos esporádicos. El objetivo de este estudio fue describir las características clínicas, epidemiológicas y ecológicas de la melioidosis en Antioquia, Colombia. Métodos: Se trata de la descripción de una serie de casos. Resultados: Se identificaron siete casos. Se aisló Burkholderia pseudomallei de sangre periférica, líquido pleural y orina y se identificó mediante el sistema automatizado VITEK 2 (bioMe'rieux) y el kit bioquímico API 20NE. Cinco de los casos presentaban una forma bacteriémica con shock y compromiso pulmonar y dos de estos pacientes fallecieron. Los casos de melioidosis no bacteriémica presentaban compromiso genitourinario, abdominal y osteoarticular. Todos los pacientes tenían comorbilidades y vivían en zonas rurales cálidas y húmedas de la región centro-occidental de Colombia (Antioquia). La diabetes mellitus, la insuficiencia renal y otras enfermedades crónicas son factores de riesgo importantes para el desarrollo de formas graves. Conclusiones: Los casos aquí presentados son similares a los que ocurren en áreas endémicas en cuanto a comorbilidad, factores de riesgo, presentación clínica y condiciones ambientales.
