This study analyses the characteristics of institutional development and community participation as main aspects of decentralization. The findings show that in spite of the fact that the three municipalities studied (Itagüí, La Ceja and Balboa) have different levels of social and economic development, they have similar advances in implementing decentralization and have faced many obstacles such as the lack of political will of mayors to support the process, the lack of technical capacity of the staff of local health head quarters and their constant changes as a consequence of political lobbying and the complexity of legislation. The information system also showed low quality and effectiveness indicators in making decisions. |

The information system also showed low quality and effectiveness indicators in making decisions.RESUMEN: La descentralización del sector salud implica cambios de orden fiscal, político, social y administrativo para que los municipios asuman las competencias transferidas por el gobierno central y departamental con lo cual el gobierno municipal es responsable de la salud de la población. Este estudio analiza las características del desarrollo institucional y de la participación comunitaria como elementos claves de la descentralización. Los resultados señalan que a pesar de que Itagüí, La Ceja y Balboa son tres municipios con niveles de desarrollo socioeconómico muy diferentes, tienen en general avances similares en la implementación de la descentralización y han enfrentado obstáculos tales como la falta de voluntad política de los alcaldes de cada periodo para apoyar el proceso, la falta de capacidad técnica del personal de las direcciones locales de salud, la alta rotación de estos como expresión del clientelismo político y la complejidad de la legislación. El desempeño de los organismos de dirección del sector es bajo y los mecanismos de coordinación entre ellos son deficientes o inexistentes; el sistema de información en salud obtuvo igualmente bajos índices de calidad y efectividad en la toma de decisiones.
