A surveillance system requires developing the functional ability to compile, analyze, and disseminate data in a timely fashion to those able to undertake effective prevention and control actions. In public health, it is imperative to direct surveillance actions not only to health problems of infectious or chronic origin, but also those caused by external agents. These agents can be closely related to the environment, which has been deteriorating at an increased pace over the last decades. In the case of pesticides, the Pan American Health Organization (PAHO) established as a priority promoting better methods for collecting data related to acute pesticide poisoning (APP) in member countries. The aim is to better understand health problems caused by pesticides, the magnitude of which are still not known with precision. This document presents some preliminary guidelines proposed and adopted by PAHO on this subject. An APP surveillance system makes it possible to determine how poisoning is affecting the population’s health, population groups most affected, types and characteristics of high-risk exposures, main pesticides involved, and other determining factors. This information is used to direct prevention and control actions to reduce the negative health effects of chemical substances in places where cases are identified (i.e. in practical terms, an epidemiological blockade). Furthermore, if surveillance system data are crosschecked with complementary information from other sources, surveillance can facilitate the identification of pesticide use patterns and evaluation of contaminated soils, water and pesticide residues in food.

Un sistema de vigilancia requiere desarrollar la capacidad funcional para recopilar, analizar y difundir datos de forma oportuna a las personas capaces de emprender acciones de prevención y control eficaces. Es un imperativo en salud pública orientar las acciones de vigilancia no sólo a problemas de salud de origen infeccioso o crónico, sino también desarrollar este tipo de estrategias sobre eventos de salud originados por agentes externos e íntimamente relacionados con el ambiente, cuyo deterioro se ha venido acrecentando de manera acelerada durante las últimas décadas. Para el caso de los plaguicidas, la Organización Panamericana de la Salud (OPS) ha establecido como prioritario promover en los países mejores métodos para recolectar datos relacionados con este tipo de intoxicaciones agudas con miras a tener una mejor comprensión de esta problemática, cuya magnitud real aun no se conoce con precisión. Este documento presenta algunos lineamientos preliminares propuestos y adoptados por la OPS en este tema. En las intoxicaciones agudas producidas por plaguicidas (IAP) un sistema de vigilancia permite determinar, cómo este tipo de intoxicaciones está afectando la salud de la población, los grupos de personas mas implicadas, el tipo y las características de las exposiciones de mayor riesgo, los principales plaguicidas involucrados y demás factores determinantes. El objetivo final es orientar acciones de prevención y control que disminuyan el efecto negativo a la salud de este tipo de sustancias químicas en los lugares donde se identifican casos (i.e., en términos prácticos, un bloqueo epidemiológico de casos.)
