RESUMEN: A pesar del avance en nuevas terapias para el tratamiento de las enfermedades reumatológicas no se ha logrado cambiar la mortalidad a largo plazo, y en estudios recientes se ha mostrado un incremento de la mortalidad debido a enfermedad cardiovascular. Ello probablemente se da por el papel que juega la inflamación crónica en estas enfermedades, a la vez que por el uso de dosis elevadas y continuas de esteroides y otros medicamentos que se sabe incrementan el riesgo de enfermedad aterosclerótica. Para tratar de corregir algunos de los factores de riesgo de la aterosclerosis acelerada se ha venido postulando el uso de las estatinas. En los análisis de los estudios se encontró que el beneficio de las estatinas aparecía en un corto periodo de tratamiento, en donde la regresión de la placa aterosclerótica no había sucedido, por lo tanto se empezaron a explicar los beneficios por efectos adicionales o pleiotrópicos. Algunos de estos efectos afectan la aterogénesis e incluyen una mejoría en la función de la célula endotelial, disminución del proceso inflamatorio, inmunomodulación, entre otros beneficios. Todos estos efectos nos ayudarían en el control de las enfermedades reumáticas y, teniendo en cuenta la mortalidad por enfermedad aterosclerótica, podrían ser una de las herramientas en su manejo. |

In recent studies, an increment in the mortality due to cardiovascular disease has been demonstrated, probably because of the role that chronic inflammation plays on rheumatic pathologies, and, at the same time, because of the use of high doses of steroids and other medications that are known to augment the risk of atherosclerotic disease. Trying to correct some risk factors for accelerated atherosclerosis, the use of statins has been taking place in the current therapy. In the analysis of the studies, the benefit of the statins was found on a short treatment period, in which the reduction of the atherosclerotic plaque had not happened yet. Because of this, the benefits started to be explained as a consequence of additional or pleiotropic effects. Some of the above mentioned effects, affect the atherogenesis and include an improvement in the endothelial cells function, a reduction in the inflammatory process and the immune-modulation among others.
