Methods: A retrospective review was undertaken to identify all patients undergoing primary palatoplasty between 2017 and 2019. Patients who developed oronasal fistula were included (cases) and compared with patients from the same cohort without fistula (controls). Demographic, clinical, and perioperative variables and their association with postoperative fistula presentation were explored through crude and adjusted analysis. Results: 139 patients with a median age of 6 years (5-6) who underwent primary palatoplasty were found. Forty-five presented oronasal fistula, corresponding to 32% of the population, and 64% required subsequent repair of an oronasal fistula. In the crude analysis, the surgeon's experience (OR 0.44) was associated significantly with the outcome. Adjusted analysis showed an association between surgical site infection and syndromic presentation. Conclusions: The experience of the surgeon is a protective factor for the oronasal fistula presentation; in addition, the presentation of infection of the operative site, the syndromic presentation, and the clinical follow-up at one year were also relevant; the latter, possibly due to the social context of our patients.

Adicionalmente, en más la mitad de los pacientes implica la realización de cirugías adicionales que aumentan el costo al sistema de salud y los riesgos para el paciente. En este estudio de casos y controles se buscó determinar cuáles son los factores de riesgo específicos para la fístula oronasal después de palatoplastia primaria en la población de estudio. Métodos: Se realizó una revisión retrospectiva para identificar todos los pacientes llevados a palatoplastia primaria entre 2017 y 2019. Los pacientes que desarrollaron fístula oronasal fueron incluidos (casos) y se compararon con pacientes de la misma cohorte que no habían tenido dicho desenlace (controles). Se exploraron factores demográficos, clínicos y las variables perioperatorias y su asociación con la presentación de fístula postoperatoria por medio de un análisis crudo y ajustado asociados con el desenlace. Resultados: 139 pacientes llevados a palatoplastia primaria, de los cuales 45 presentaron fístula oronasal, con una incidencia de período del 32% y una tasa de cirugía secundaria del 64%. Dentro del análisis univariado se se encontraron como factores de riesgo estadísticamente significativos la experiencia del cirujano, la infección de sitio operatorio, la presentación sindrómica y el seguimiento a 1 año. En el análisis multivariado la infección de sitio operatorio y la presentación sindrómica.
