RESUMEN: Documentar y analizar las experiencias de la ciudad en cuanto a mecanismos de participación social en salud y la generación de acuerdos o alianzas para la ejecución de programas o acciones de salud pública desde la perspectiva del capital social. Métodos: se recolectó información primaria mediante unas encuestas semi-estructuradas a 136 líderes en temas de salud en la ciudad y se analizó información y datos de la Encuesta de Percepción Ciudadana de Medellín Cómo Vamos 2009. Resultados: La participación social en salud ha tenido un aumento importante, particularmente en los Presupuestos Participativos. Pero la medición del capital social también involucra componentes como la confianza, normas y valores cívicos. Aunque la muestra de la encuesta no tuvo los criterios técnicos de representatividad, sí permite aproximar evidencias sobre algunas condiciones de un tipo de “capital social” en el sistema local de salud. De allí se desprende que sobre cultura ciudadana y fortalecimiento de lazos de cohesión social y cooperación en pro del bien común, es necesario avanzar más. Conclusiones: Los líderes en salud en Medellín mantienen un nivel de “confianza en estado de alerta” y perciben irrespeto por las normas cívicas. La credibilidad hacia algunas instituciones de la salud en la Ciudad es relativamente alta, pero la confianza hacia la Policía Nacional es baja. La violencia que experimenta algunas comunas limita o inhibe la generación de confianza. La participación en organizaciones y redes sociales ha venido aumentado su nivel de credibilidad y capacidad, pero se percibe que ésta no se tiene en cuenta en la toma de decisiones. Los Presupuestos Participativos han tomado importancia como mecanismo de participación en los últimos años. No obstante, algunas organizaciones se encuentran atravesadas por problemas de comunicación, conflicto de intereses y falta de compromiso. |

Methods: Primary data were collected through semi-structured survey of 136 leading health issues in the city and analyzed information and data from the Citizen Perception Survey 2009 “Medellín Cómo Vamos”. Results: Social participation in health has been a significant increase, particularly in the Participatory Budget. But the measurement of social capital also involves components such as trust, civic norms and values. Although the survey sample did not have the technical criteria of representativeness, they can bring evidence on some conditions of a type of “social capital” in the local health system. From there it follows that on civic culture and strengthening social cohesion and ties of cooperation for the common good, more progress is needed. Conclusions: The health leaders in Medellin maintain a level of “confidence in a state of alert” and perceived lack of respect for civic norms. The credibility to some health institutions in the city is relatively high, but confidence in the national police is low. The violence experienced by some municipalities restrict or inhibit the creation of trust. Participation in organizations and social networks has been increasing its level of credibility and capability, but is perceived to be not taken into account in decision-making. Participatory Budgets have become important as a mechanism for participation in recent years.
