This scoping review identifies key methodological aspects for the effective treatment of specific phobias through virtual reality exposure therapy (VRET). Five randomized clinical trials (n = 316) published between 2019 and 2025 were analyzed, selected through a systematic search in PubMed and ScienceDirect. Using the DIAD tool, central elements were identified, including progressive hierarchization, exposure frequency and duration, sample size, psychoeducation, gamified elements, multisensory stimulation, and control of virtual realism and immersion. The findings provide a technical and clinical framework to optimize VRET planning, enhancing its effectiveness and clinical applicability despite the limitations of this emerging technology.1.
