The objective of the study is to focus on the existence of the moral hazard in the contributory system, which generates a shared payment mechanism (co-payment, moderator fee, among others) and that affects in particular the conditions of equity of affiliated persons, in addition to generating inefficiency in the operation of the health services. Throughout this text it is shown briefly, the theoretical debate whether or not to the establishment of the of co-payments mechanism to access medical services in general.Este artículo revisa el problema riesgo moral, considerado uno de los fallos más importantes del mercado de la salud en los contextos internacional y nacional y, en este caso, porque afecta en alguna medida el régimen contributivo del sistema general de seguridad social de salud de Colombia. Este régimen genera el mecanismo de pagos compartidos e incide de forma particular sobre las decisiones de afiliación, demanda y uso de servicios médicos, las cuales terminan por generar efectos sobre los niveles de bienestar en la población, además de producir ineficiencia en su funcionamiento. A lo largo del texto se analiza la conveniencia o no del mecanismo de copagos para acceder a los servicios médicos en general.
