Impacto cognitivo de la terapia electroconvulsiva de continuación y mantenimiento en pacientes con trastorno bipolar y trastorno depresivo mayor: una revisión sistemática de la literatura610 - Medicina y salud::616 - Enfermedades3. Metodología: revisión sistemática realizada según la guía PRISMA 2020. Se incluyeron ensayos clínicos aleatorizados y estudios observacionales que evaluaran la cognición mediante instrumentos neuropsicológicos validados. La búsqueda se realizó en múltiples bases de datos hasta julio de 2025. Los resultados se sintetizaron de forma narrativa. Resultados: se incluyeron 8 estudios con diferentes diseños metodológicos. En ensayos clínicos y estudios longitudinales no se evidenció deterioro cognitivo global, observándose estabilidad en medidas como el MMSE y el MoCA, incluso tras exposiciones prolongadas. Algunos estudios reportaron mejorías en dominios específicos. Un estudio transversal identificó déficits en funciones ejecutivas y memoria de trabajo, no reproducidos en estudios longitudinales. Conclusiones: la terapia electroconvulsiva de mantenimiento presenta un perfil de seguridad neurocognitiva favorable. La individualización de los protocolos, el uso de parámetros técnicos modernos y el monitoreo cognitivo seriado son claves para optimizar su seguridad y aplicación clínica.Objective: to synthesize and analyze the available evidence on the cognitive impact of continuation and maintenance electroconvulsive therapy in patients with major depressive disorder and bipolar disorder. Methods: systematic review conducted according to PRISMA 2020 guidelines. Randomized controlled trials and observational studies assessing cognition with validated neuropsychological instruments were included. The search was performed across multiple databases up to July 2025. Results were synthesized narratively. Results: eight studies with different methodological designs were included. Randomized trials and longitudinal studies showed no evidence of global cognitive deterioration, with stability observed in measures such as MMSE and MoCA, even after prolonged exposure. Some studies reported improvements in specific domains. A cross-sectional study identified deficits in executive functions and working memory, which were not replicated in longitudinal designs. Conclusions: maintenance electroconvulsive therapy demonstrates a favorable neurocognitive safety profile.
